Thank you for choosing our online request for services function. Below, you will find our new assignment form for both structure and vehicle fires. Please fill in your contact information, followed by the relevant fields regarding your new origin and cause assignment.

The office staff is available to accept new assignments 24-hours-a-day by filling out the form below. The assignments submitted online will be inputted into our system and assigned to an investigator immediately. Assignments received during the weekend or after business (Business Hours 7:00 a.m.-5:00 p.m.)hours will be inputted and assigned the morning of the next business day.

Assignments may also be phoned in, faxed in or even dropped off at the office. Please refer to the "Contact Us" tab to obtain contact information. If you are interested in getting in touch with a specific investigator, please refer to the "Investigator" sub-tab found under "Staff."
